Free Summer Meals

Family Choices, Inc. is participating in the Summer Food

Service Program. Meals will be provided to all children 18 yrs of age & younger without charge. 

Acceptance and participation requirements for the program and all

activities are the same for all regardless of race, color, national origin,

sex, age or disability, and there will be no discrimination in the course of

the meal service.  Meals will be provided

at sites listed as follows:  No Limits Christian Church Monday 6/2/2014 - Friday 7/25/2014 Breakfast 8-9am, Lunch 12-1pm. Cornerstone Baptist Church Breakfast 7:30-8:30am, Lunch 11-1pm.

If you wish to file a Civil

Rights program complaint of discrimination, complete the USDA Program

Discrimination Complaint Form, found online at

http://www.ascr.usda.gov/complaint_filing_cust.html, or at any USDA office, or

call (866) 632-9992 to request the form. You may also write a letter containing

all of the information requested in the form. Send your completed complaint
